If you find you're missing the 13th (//013) Hostiles Databank Entry, you need to defeat an Infected Scout. It appears randomly, but you can increase your chances by finding fallen scouts. This requires you to play online so that you can retrieve the data of other players who have died. Look for an Unknown suit signal throughout your playthrough. Scan it for a chance to battle an Infected Scout.
