If you've collected all the proper items (Ciphers, Audio Logs, and Xeno-Archives) and a Survey Trophy notification did not pop-up, try the following: 1. Go into the Menu > Settings. 2. Go to the Gameplay tab. 3. Select Start Online and switch to Disabled. 4. Leave the Menu. 5. Close the game. 6. Start Returnal again offline.
