Once you defeat Mephisto and finish The Guardian quest of Act 3, you must talk to Deckard Cain. He mentions that to progress you need to enter a Portal. The Portal is located in the same dungeon you defeated Mephisto, Durance of Hate Level 3. You'll find it near the staircase glowing in red near the location where you fought Mephisto.
